Back to School PCF Online Forms and New Information

Dear Parents and Guardians,

At the beginning of each school year, there are parent consent forms that must be completed by the first day of school. These forms MUST be completed by a parent/guardian in order to access clinic services, health screenings and other student services.  For example, if you think your student might need a band-aid from the clinic, or if you want the school to be able to screen your child's hearing you need to give consent.

This year, the forms are digital and are located on Skyward.  Please log into https://parents.classlink.com/ocpsto access the Parent Portal and locate the Skyward app. Please let classroom teachers know if you would prefer to complete a hardcopy of the forms.

OCPS families that have NEVER logged into the OCPS Parent Portal before, please view this video to register. If an error message is received, please complete the OCPS Parent Portal Help Form. 

Once you are in Skyward, you will see two forms on the wall of the dashboard. The completion of the 2024 PCF forms are required.   Click on the link “Fill Out Online Form for (student name)” under the 2024 PCF section. Proceed to complete the 5 forms on the right side of the screen.

 The Parent Consent Form includes fields for:  

  • Acceptable and responsible uses of technology 

  • High school social media use 

  • District use of a student’s phone and/or video 

  • Release of student directory information 

  • Clinic services 

  • Health screenings 

This year, parents/guardians must choose to opt in or out of each individual clinic service and health screening by law. In the case of an emergency, parent/guardian and appropriate emergency personnel will be contacted. 

New Technology Fee Schedule

 

OCPS is proud to provide ALL K-1 students with and iPad and all Grades 2-12 students with a laptop! When an iPad/laptop or accessory is damaged, an incident fee is assessed. The chart below shows the device fee structure. If a non-warranty repair be completed, the incident fee will be applied to the cost of the repair. However, if the repair falls under warranty and there is no cost to repair, the incident fee will be refunded.

Student/Parent/Guardian Responsibility for the digital device repair/replacement cost

1st incident = $25

2nd incident = $50

3rd (and forward) incident = Depreciated value (see below)

Depreciated value

The fee structure now accounts for a four-year depreciation schedule of the device (each year ends June 30). The depreciation schedule - years from purchase:

1 year = Cost of repair or replacement

2 years = $400

3 years = $250

4 years = $100

Student/Parent/Guardian Responsibility for the accessory replacement cost

The accessory fee structure is now itemized based on accessory type (charger or protective case). The new protective cases for devices in grades 6-12 are considered accessories.

1st incident = $10

2nd incident = $15

3rd (and forward) incident = $25


 

The Device Refusal Form on Skyward is OPTIONAL and only needed for families planned to have their student use personal device, NOT a district device. 

Device refusal and Bring Your Own Device

Parents’ and guardians have a right to refuse the use of a district provided take-home device in grades 3-12 and bring their own device to school. To Bring Your Own Device, a parent must provide a device that meets certain guidelines and complete the Device Refusal Form on the OCPS Parent Portal in the Skyward app. Guidelines and form instructions can be found atbyod.ocps.net.
